If you need a copy of your patient care report, simply download and fill out the medical record request form, then return it to us. Our billing staff will then be able to assist you.

Inter-Facility Patient Transfers

Advanced Life Support (ALS) and Basic Life Support (BLS) Transport Services Available

When a higher level of care is needed, we can transfer to medical facilities within 100 miles. Factors such as medical necessity, staff availability and weather can play a role in whether we will accept a transport. Please view our transfer guidelines prior to making your request.
